twistedには、「（考え方や性格などが）ゆがんだ、ひねくれた」という意味があります。
What has got you twisted?は、直訳すると「何があなたをひねくれさせたの？」となります。
a slap on the wrist
slap on the wristは直訳すると「手首への平手打ち」となりますが、「軽いお仕置き」、「緩やかな罰や警告」のことを言います。
本来ならばもっと重い罰を課すべきなのに、軽いお仕置き程度で済むというニュアンスがあります。
